All clinical services are provided solely by or under the supervision of licensed healthcare professionals. Services are available to patients who have established a patient-provider relationship with Peak Vitality ABQ through a formal intake and evaluation process.

This website serves as an informational resource and as a means of initial contact. Visiting this website does not create a patient-provider relationship.

Appointments & Cancellations

Scheduling

Appointments may be requested through our website, by phone at (505) 886-1010, or by email at Clopez@peakvitalityabq.com. Submitting a consultation request through our website does not guarantee an appointment; all requests are subject to availability and confirmation by our staff.

Cancellation Policy

We ask that you provide at least 24 hours' advance notice if you need to cancel or reschedule an appointment. This allows us to offer your time slot to other patients who may be waiting for care.

Late Arrivals

If you arrive more than 15 minutes late for a scheduled appointment, we may need to reschedule your visit to ensure adequate time for your care. We will do our best to accommodate you within the same day whenever possible.

New Patient Intake

New patients are required to complete all intake forms prior to or at the time of their first appointment. Incomplete intake documentation may result in rescheduling. Our staff will communicate all required forms at the time of booking.

Payment & Billing

Payment for Services

Peak Vitality ABQ operates as a private practice. Payment expectations, fees, and scheduling policies are communicated during the inquiry and booking process. Unless another arrangement has been made in advance, payment is generally due at the time services are provided.

We accept major credit and debit cards, and other payment methods as posted in our office or communicated by our staff.

Insurance

If you have questions about insurance, reimbursement, or documentation for your own records, please contact the practice directly before your appointment so the administrative process can be explained clearly.

Pricing & Fee Changes

Service fees are subject to change. Current pricing will be communicated to you prior to scheduling or at the time of your appointment. We encourage you to contact our office to discuss fees for any specific services you are considering.

Refund Policy

Fees for services already rendered are generally non-refundable. Deposits or other advance payments may be subject to the specific terms communicated at the time of booking or purchase. Please contact the office with any billing concerns.

Collections

Accounts with outstanding balances may be subject to collection procedures, including referral to a collections agency, in accordance with applicable law. You agree to be responsible for all reasonable costs of collection, including reasonable attorneys' fees, to the extent permitted by New Mexico law.

Jurisdiction & Venue

You agree that any legal action or proceeding arising out of or relating to these Terms or your use of the Site or services shall be brought exclusively in the state or federal courts located in Bernalillo County, New Mexico. You consent to the personal jurisdiction and venue of such courts.

Medical Malpractice

Claims arising from the professional provision of medical or healthcare services are subject to the New Mexico Medical Malpractice Act (NMSA 1978, §§ 41-5-1 et seq.) and its applicable procedural requirements, including any required use of the Medical Review Commission. Nothing in these Terms waives your rights or our obligations under applicable medical malpractice law.
